When Sultana fatally injured Kokani to the point she though he was dead, Kokani obviously lost interest in her. This is despite him trying the much he could to win her love but she continually expressed her feelings to JJ and openly confessed her love to JJ, something which hurt Kokani to the core.
Thus when he managed to escape and miraculously recovered, he never made any efforts to persue her nor to reveal himself to the family anymore, but he went ahead with his career. But then, he resurfaced again to operate Sultana and it is evident that he still values her. This is because he successfully and skillfully operated her to ensure she manages to see for the very first time.
He came back to visit her and carried along some oranges for her. But to his surprise, Sultana shouts at him soon he enters her room, she actually wanted him to speak up but he remained silent. When she throbbed further, her was shocked by her behavior, left the oranges by the table and while weeping, he fled away.
