According to the report, part of the coach’s truss bar hanger struck a crossing block and became lodged in the nose of the crossing, causing the coach to be pulled and a wheel to climb onto the rail head.

The incident resulted in the derailment of 14 coaches, which came off the tracks on both the up and down lines, the joint investigation report said.

The remaining section of the truss bar hanger was recovered from the accident site, while other components were also found lodged in the crossing.

The train had departed with a total load of 672 tonnes at 5 AM, according to the report.

It added that the reception and departure signals were green when the train entered the yard.

The engine and luggage van passed through the crossing safely before the component beneath the second coach became stuck, leading to the derailment.

An assessment of the damage caused to various railway departments is under way, the report said.
