Here’s What Matters. Two healthcare giants drop their third-quarter numbers on the same morning, and what each reveals about drug costs, insurance margins, and guidance ranges will set the tone for the entire sector heading into 2027.

Johnson & Johnson (NYSE:JNJ | JNJ Price Prediction) and UnitedHealth Group (NYSE:UNH) both report third-quarter results before the opening bell on Tuesday, October 13, 2026. One is a drug-and-device maker and the other a managed-care giant, so the morning will test the entire healthcare sector at once.

Johnson & Johnson posts its release at approximately 6:45 a.m. ET and holds its call at 8:30 a.m. ET. UnitedHealth Group’s call starts at 8:00 a.m. ET.

On October 2, Jim Cramer noted that investors are avoiding defensive names: “They don’t want to buy the safety of a J&J, which I still think is what you should be buying here.” The price data shows shares up 26.1% year to date and 36.0% over the past year. The only dip is a 1.8% slide over the past month.

Last quarter, adjusted EPS of $2.90 beat estimates on revenue of $25.31 billion. Innovative Medicine grew 7.8% and MedTech rose 4.5%. Management also raised full-year adjusted EPS guidance to $11.60 to $11.75.

Stelara fell 55.2% on biosimilar competition, while Tremfya rose 72.5%. Expect questions on Firefly Bio, which guidance leaves out, as well as Ottava’s FDA path and Abiomed’s 2% decline. The full-year EPS consensus of $11.0734 falls below the guided range of $11.60 to $11.75.

UnitedHealth shares are up 12.8% year to date but down 1.1% over the past month. Analysts have revised their estimates higher anyway. Third-quarter EPS consensus rose to $4.1495 from $3.7557, 90 days ago.

Second-quarter EPS reached $6.38 as the medical care ratio improved to 86.7% from 89.4%. That figure included $860 million of positive reserve development. Full-year guidance calls for adjusted EPS of $19.50 to $20.00 and a medical care ratio of 88.1%.

Medicare cost trend ran below plan. Commercial trend is running “modestly above 11%,” and management says this pushes full commercial margin recovery past 2027. Medicare Advantage membership has shrunk by 965,000 since year-end. Optum Health revenue fell 5% on about 700,000 fewer value-based care patients.

Drug spending links the two companies. UnitedHealth named specialty and anti-inflammatory drugs among its commercial cost drivers, and that category includes J&J’s immunology franchise. Tremfya growth counts as revenue for J&J and as cost pressure for payers.

Beyond that, the link is thin. J&J’s results depend on launches and patent cliffs. UnitedHealth’s depend on use, pricing, and reserves.

For J&J, look for any change to the $11.60 to $11.75 range, or Stelara erosion worse than 55.2%. For UnitedHealth, check whether the 88.1% medical care ratio target holds and whether commercial trend moves further above 11%. Those four markers will indicate which story is driving the healthcare sector into 2027.
