MIKHAIL MORDASOV | AFP via Getty Images Share on Twitter Share on Facebook Share on Pocket The Erie County Department of Health has confirmed a travel-related case of diphtheria and identified four schools in the West Seneca Central School District as possible exposure sites, according to Buffalo Toronto Public Media. Letters went to families at Clinton Primary, Northwood Elementary, Winchester-Potters Elementary and East Middle School. Households are being asked to watch for symptoms through Oct. 18.

School and health officials have stressed that the chance of infection is extremely small. Northwood Elementary principal Angela Ferri described the risk that way in an Oct. 9 letter to families, The Buffalo News reported.

Still, diphtheria is so uncommon in the United States that a single confirmed case sets off a full public health response. For parents in West Seneca and across the Buffalo area, the practical questions are simple: Is my child up to date on DTaP or Tdap, and do I know the warning signs?

The county has not said whether the patient is a student, a staff member, or someone else connected to the schools. It also has not said which form of the disease was identified. Diphtheria can affect the nose and throat, known as respiratory diphtheria, or the skin, known as cutaneous diphtheria.

The health department said it is acting “based on an abundance of caution” and working with district leaders to inform students, staff and families. It is also reviewing student immunization histories with school nurses. Students found to be unvaccinated or under-vaccinated may receive additional notices, according to the report.

Households were asked to make sure students, staff, and family members are current on diphtheria vaccines. That includes DTaP for young children and Tdap or Td for older children and adults.

The Oct. 18 date has a purpose. Symptoms usually appear two to five days after infection, with a range of one to 10 days, according to the New York State Department of Health. A monitoring window that extends past the latest likely exposure gives families and doctors time to catch any late case.

Only 10 diphtheria cases have been reported in the United States over the past 20 years, according to local reports, and the CDC says the last confirmed U.S. case of respiratory diphtheria was in 1997. The CDC describes diphtheria as uncommon in the country and usually tied to international travel. Most U.S. cases involve people who are unvaccinated or not fully vaccinated, especially those who travel to areas where the disease circulates or have close contact with travelers, New York health officials say.

That travel link is why one case in Western New York draws attention. The bacteria still circulate in parts of the hemisphere, and travelers can carry them home without knowing it. In June, the Pan American Health Organization reported 163 confirmed diphtheria cases and five deaths in the Americas in the first 21 weeks of 2026, more than double the number for the same period in 2025. Haiti accounted for 159 of those cases and all five deaths, with cases also reported in Brazil and Peru.

PAHO said more than half of this year’s cases were in people who were unvaccinated or had unknown vaccination status. Regional coverage for the third dose of the DTP vaccine fell to 87% in 2024, below the 95% target for community protection. “Vaccination is key to preventing cases and outbreaks,” the organization said in its alert.

The respiratory form is the dangerous one. A toxin made by the bacteria can create a thick gray coating in the nose and throat that makes breathing and swallowing difficult. Even with treatment, about 1 in 10 people with respiratory diphtheria die, and without treatment up to half do, according to the CDC’s symptom guidance. Skin infections rarely cause severe illness, though the sores heal slowly.

The CDC lists sore throat, mild fever, swollen neck glands, weakness, and a gray coating in the nose or throat as the most common signs of respiratory diphtheria. Skin infections may appear as an open sore with a clear edge, a scaly rash, redness, swelling, or pain.

Families connected to the four schools who notice these symptoms should call a health care provider right away, explain the possible exposure, and keep the person home until a clinician says otherwise. Trouble breathing, noisy breathing, or severe neck swelling requires emergency care. Diphtheria is treated in the hospital with antitoxin and antibiotics, and public health officials decide whether close contacts need preventive treatment.

Checking vaccine records is the most useful step for everyone else. Children typically receive five DTaP doses between 2 months and 6 years old and a Tdap booster at 11 or 12, and adults need a Td or Tdap booster every 10 years, according to New York health officials. New York requires diphtheria vaccination for children in pre-kindergarten programs and schools.

Parents can get records from their pediatrician or school nurse. Adults who cannot remember their last tetanus shot are likely due for a booster, and the same quick shot also restores diphtheria protection. Uninsured families can ask the county health department about low-cost vaccine options, and children may qualify for free shots through the federal Vaccines for Children program.

Officials have not reported any additional cases. The county is expected to contact families directly if new cases or exposures are identified, and MedicalDaily will update this story if the case count or the guidance changes.

The Erie County Department of Health confirmed one travel-related diphtheria case and named Clinton Primary, Northwood Elementary, Winchester-Potters Elementary and East Middle School as possible exposure sites.

School officials described the risk of infection as extremely small. Officials have not reported any additional cases.

How long should families watch for symptoms?

Families connected to the four schools were asked to monitor for symptoms through Oct. 18.

Respiratory diphtheria can cause sore throat, mild fever, swollen neck glands, weakness, and a gray coating in the nose or throat. Skin diphtheria can cause slow-healing sores, rash, redness, and swelling.

It spreads mainly through respiratory droplets from coughing or sneezing, and through contact with an infected person’s open sores or items that touched them.

Make sure children are current on DTaP or Tdap and that adults have had a Td or Tdap booster within the past 10 years.

What should I do if my child develops symptoms?

Call a health care provider right away, mention the possible school exposure and keep your child home. Seek emergency care for trouble breathing or severe neck swelling.
