e5tfeyi3\">A coroner has criticised a university over failing to respond to a student\u2019s \u201ccry for help\u201d weeks before he took his own life.<\/b><\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Harry Armstrong Evans, 21, died at his family home in Cornwall in June 2021 after suffering a mental health crisis at the University of Exeter.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Assistant Cornwall coroner Guy Davies recorded a conclusion of suicide.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">The university said it would \u201cconsider the coroner\u2019s detailed conclusions\u201d.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Mr Davies said he would write a report to the university with areas of \u201cconcern\u201d to prevent future deaths.<\/p>\n<\/div>\n<\/div>\n<p><h2 death was due to an acute mental health crisis which was preceded by a catalogue of missed opportunities along with system failures which together led to an absence of proactive results which meant Harry could not receive support.\u201d<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Harry had been in the third year of an astrophysics degree when he took his own life at his family home in Launceston.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Rupert Armstrong Evans, Harry\u2019s father, said outside the hearing: \u201cHarry was a beautiful, kind, intelligent and gentle soul. He never had a cross word with anyone.\u201d<\/p>\n<\/div>\n<\/div>\n<div data-component=\"image-block\" class=\"ssrcss-18mjolk-ComponentWrapper ep2nwvo1\">\n<figure class=\"ssrcss-wpgbih-StyledFigure ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Harry outlined his struggles, isolation during the Covid 19 pandemic \u201cwith no human contact\u201d which had had \u201can adverse impact on my mental health\u201d, along with concerns about poor exam results and financial problems.<\/p>\n<\/div>\n<\/div>\n<div coroner questioned whether the university\u2019s wellbeing team\u2019s case management system was \u201cfit for purpose\u201d, after two phone call logs made by Harry\u2019s mother raising concerns about his welfare in May 2021 had been accidentally deleted.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">The hearing in Truro was told the wellbeing team had since requested a new system and had been operating a \u201cworkaround\u201d to avoid the mistake reoccurring.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Mr Davies said: \u201cIn Harry\u2019s case the safety net did not operate to safeguard Harry. <\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">\u201cConcerns were raised on a number of occasions by both Harry and his parents.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">\u201cMy central finding will be that the welfare service did not proactively respond to those concerns and did not provide the necessary support for Harry.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">\u201cSpecifically, there was no engagement with Harry\u2019s family.\u201d<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">He said he would be writing a preventing future deaths report to the university\u2019s vice-chancellor, which would highlight his concerns.<\/p>\n<\/div>\n<\/div>\n<p><h2 id=\"Compassionate-and-supportive\" class=\"ssrcss-y2fd7s-StyledHeading e1fj1fc10\"><span role=\"text\">\u2018Compassionate and supportive\u2019<\/span><\/h2>\n<\/p>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">Mike Shore-Nye, registrar at the University of Exeter, said: \u201cWe will consider the coroner\u2019s detailed conclusions in this case and make sure we learn the lessons to enhance our support and operations further, specifically in the areas recommended by the coroner.\u201d <\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">He said the university had \u201cinvested significantly in student welfare and wellbeing support in recent years\u201d. <\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">This included setting up a \u201cbespoke partnership with local NHS and support services for students with complex mental health needs, depression and anxiety, with student support services being available seven days a week, including throughout the Covid-19 pandemic\u201d.<\/p>\n<\/div>\n<\/div>\n<div data-component=\"text-block\" class=\"ssrcss-11r1m41-RichTextComponentWrapper ep2nwvo0\">\n<div class=\"ssrcss-7uxr49-RichTextContainer e5tfeyi1\">\n<p class=\"ssrcss-1q0x1qg-Paragraph eq5iqo00\">He added: \u201cOn education, during the pandemic all students were supported with a no disadvantage guarantee to ensure their assessment marks didn\u2019t suffer as a result of the difficult circumstances surrounding the pandemic.
